Here at Corti, we want to make it easy for organizations to take unstructured (or structured) text and convert it into accurate, structured documents. This helps clinicians, support teams, and operations staff spend less time writing and more time focusing on what matters.
Here are the three most common ways organizations use the API for document generation:
Transcript → Document
When to use it
Organizations that record or transcribe conversations (for example, between a clinician and patient, or an agent and customer) often want those conversations transformed into structured documentation. Instead of manually writing up notes afterwards, the Corti API can take a transcript and generate a complete report or note.
Example
A healthcare provider uploads the transcript of a patient consultation and instantly generates a SOAP note or discharge summary.
A call center uses recorded transcripts to generate follow-up letters to customers.
How it works with the API
Provide the transcript text as the input.
Choose a template (like “SOAP Note” or “Clinic Summary”).
The API returns a structured document with the right sections filled in.

When to use it
Sometimes you don’t have a full transcript—just unstructured text, bullet points, or rough notes. Corti can turn this raw text into a polished document in your organization’s preferred format.
Example
A clinician jots down short notes after a consultation (“patient reports mild chest pain, normal vitals”) and the API generates a professional-looking medical note.
An HR department pastes in feedback from an employee survey and generates a summary document.
How it works with the API
Send your text (notes, draft, or summary) as input.
Select a document template.
The API organizes and rephrases the text into a clear, structured document.

Facts → Document
When to use it
Many organizations use Corti’s FactsR™ engine to capture key information (symptoms, vitals, demographics, action plans, etc.) in real time. Instead of rewriting these facts later, the API can directly use them to generate a document.
Example
During a patient call, Corti automatically extracts facts like “32-year-old female,” “itchy rash for one week,” “allergic to pollen.” At the end, a SOAP note is generated with those details already structured in the right sections.
An emergency dispatch center uses facts collected during 911 calls to generate incident reports automatically.
How it works with the API
Collect the structured facts captured during the interaction.
Send them to the API with a chosen template (like “SOAP Note” or “Incident Report”).
The API generates a complete document based on those facts.
